Learning Objectives

~ Supporting the patient through the whole journey of cardiac care by sharing new knowledge in cardiac technologies, research, and treatments of specific cardiac conditions with nurses and allied health professionals.

~ Supporting cardiac patients and their families with acute and chronic heart disease; considering their stress, suffering and quality of life issues.

NP Forum Champion

The Cardiovascular Health and Stroke Strategic Clinical Network (SCN) aims to improve the prevention, treatment and management of vascular disease, bringing together programs and services that care for cardiac and stroke patients and their caregivers.

We are a province-wide team of health care professionals, researchers, community leaders, patients and policy makers, which aims to reshape heart and stroke care in Alberta.


Alberta Health Services - NP Forum Supporter

Alberta Health is supportive of Patient and Family Centered Care and promoting cardiac health. This is a link to the Chronic Disease Management resource site where you can find educational resources for both patients and professionals.

Alberta Stroke Action Plan

The Alberta Stroke Action Plan has made a huge impact on patients who have had a stroke. The attached website has many useful resources for Stroke Care.

The Canadian Cardiovascular Society

Thank you to The Canadian Cardiovascular Society for sharing the pocket guidelines. There are several available on line and as smartphone apps.

NP Forum Chamions - Canadian Diabetes Association

The Canadian Diabetes Association's Clinical Practice Guidelines for the Prevention and Management of Diabetes in Canada represent the best and most current evidence-based clinical practice data for healthcare professionals. On a global stage, the Guidelines support the Canadian Diabetes Association's status as a leader in diabetes prevention and management, and help physicians guide their patients in the screening, prevention, diagnosis, care, management and education for Canadians living with type 1, type 2 and gestational diabetes. The 2013 Guidelines is also supported by a Guidelines Dissemination and Implementation Committee. The Committee produces innovative and practice relevant, tools, resources and programs for healthcare providers to be used at point-of-care, self-study and interprofessional continued professional development. The Canadian Council of Cardiovascular Nurses is an organization dedicated to advancing cardiovascular nursing through leadership, advocacy, research, and knowledge translation and help be the voice for cardiovascular nursing in Canada. There are many benefits of joining and supporting CCCN.

The Canadian Nurses Association (CNA) is the national professional voice of registered nurses in Canada. A federation of 11 provincial and territorial nursing associations and colleges representing over 151,400 registered nurses, CNA advances the practice and profession of nursing to improve health outcomes and strengthen Canada’s publicly funded not-for-profit health system.

CNA is responsible for the overall management of the only national areas of nursing practice competency certification program. Hypertension Canada offers great online resources under the Professional section of their website. CHEP Recommendations are under the CHEP section and PEPonline, a newly launched online training course that covers the diagnosis and management of hypertension.

AlbertaQuits provides convenient options for tobacco users and health professionals to address tobacco addiction with online access to tools, resources, quit/change plans, supportive community network, telephone counseling, texting service, and registration for group support called QuitCore. Included on the website is the AlbertaQuits Learning Series training options which provide health professionals with the awareness, knowledge and skill set needed to assess and implement tobacco intervention effectively in a variety of health care settings.
